DRC Visa Application Form and the Requirements of DRC Embassy 

• Passport (signed and valid at least for 3 months with 2 blank visa pages).
• Three copies of DRC Visa Application Form dualy coppleted and sighned.
• Three recent Colored Passport size photo stapled to the marked place of the application form.
• For Non-Canadian applicants - Proof of Legal Status in Canada (PR Card, Work Permit, Long-stay visa with a minimum validity of one year).
• Proof of Employment/ Canadian Company Employer Letter.
• Fligh Itinerary (copy of booking iof the trip).
• Bank statement copy (not older than 1 week)
• Copy of vaccination against yellow fever, required at the border before entering Congolese territory; 
• For Business visa – Notarized Invitation  Letter from a company in DRC; Canadian Company Business Letter biefly  introdusing the applicant and clearly stating the precise nature of the business and activities to be conducted (dates and places of the business contacts in DRC included)
• For Tourist and Family Visit visa - Notorized Invitation Letter .
• Client Order Form. Payments can be made by cash, money order or certified cheque. Prices do not include 13% HST.
