Ukraine Visa Application Form and Requirements of the Consulate of Ukraine

• Passport (signed and valid at least for 6 months with 3 blank visa pages).
• Ukraine Visa Application Form properly filled out , printed out and signed.
• Two Colored full front view passport size photographs on a white background, taken within six months of the date of application and attached in the designated space of the visa application form.
• Proof of Legal Status in Canada for Non-Canadian applicants (PR Card, Work Permit, etc.)
• Medical Insurance valid for the time staying in Ukraine  (can be provided for $80).
• Bank statement copy
• Flight itinerary (copy of booking of the trip) 
• For Tourist Visa - Original Invitation /Visa Support documents. Original Tourist Invitation can be provided for an additional $170. This Visa allows staying in Ukraine for 30 days.    
• For Transit Visa - Documents confirming the transit nature of travel (a visa to the third country if it is require, a ticket, etc. ) This visa allows transit through the territory of Ukraine within the period of 5 days.
• For Long-Term Visa -  Invitation from a dually registered Organization or Work permit. This visa can be issued for applicants who enter Ukraine with the purpose of obtaining documents permitting staying in Ukraine for the period of more than 90 days. Withing 45 days from the moment of entry to Ukraine a holder of long-term one entry visa should apply to relevant authorities in Ukraine for a permanent/ temporary resident permit.
